Just got the results from my recent bloodwork, This is pre trt, nothing in my system. Any input would be appreciated. Appointment is coming up on the 22nd. I've since started 200mg a week of test after the bloodwork, as my next bloodwork should be in a few months.

Testosterone - 15.59 nmol/L
Bioavailable Testosterone - 4.46 nmol/L
PSA - 0.282 ug/L
SHGB - 26.5 nmol/L
Hemoglobin - 142 g/L
Hematocrit - 0.413 L/L
Glucose - 5.5 mmol/L
Estradiol - 73.2 pmol/L
Prolactin - 13.3 ug/L
LDL - 4.28 mmol/L
HDL - 0.96 mmol/L
Cholesterol - 5.83 mmol/L

Your test levels are on the lower range but are still in a healthy range. The TRT will definitely help.
Your glucose levels, if that was taken when you are fasted, is at the upper limit of the normal range. I would keep an eye on it to see if it can come down slightly.
Your cholesterol panel is what needs the most work. Your LDL is slightly high, you want <3.5 mmol/L. Your HDL is slightly low. You want it to be >1.0 mmol/L. Obviously, since both your HDL and LDL are out of range, your total cholesterol isn't in the normal range (<5.2 mmol/L).

I don't know what your day to day looks like but I can do some basic recommendations. Consider dietary changes like having more fiber and less saturated fat. Hopefully you do regular exercise. Look into supplements like omega-3 (fish oil).

I’ve made a lot of diet changes in the past month, along with exercise 6 days a week. Weights 3 days cardio was upped to 6 days currently. So I’m hoping that’s reflected on the next blood panel. I’m also guilty of smoking a lot of weed, which I’m about to cut out.

I’ve been taking 8 fish oil caps a day, they’re 1000mg each. As well as black seed oil at 1000mg. I had a high blood pressure issue that’s since resolved. So I plan on continuing this as part of my daily routine.

Maybe I need to bite the bullet and pay for a proper diet plan. Appreciate your input.

On paper these blood test results don't really make you a candidate for TRT considering your testosterone levels are in-range, but I also understand it's not black & white. Sometimes guys can suffer symptoms of low testosterone while still "in-range".

Lipids definitely could use some improvement. HDL is barely below range but LDL is quite high if I'm recalling the reference range correctly. Sounds like you're working on that. Diet is the biggest factor from my experience. Cardio (gym cardio, not low intensity like walking) helps as well. Did you test triglycerides on this panel?

Yes, I did get triglycerides tested. Result 1.31 mmol/L.

I agree, based off these blood results I am still within range and it doesn't really make me a candidate for trt. As you said, its the symptoms that are killing me. IE. severe depression, anxiety, ED, fatigue, irritability and the list goes on. I find slightly higher test really helps mitigate these symptoms and allows me to live a much better QOL with less dependance on SSRI's and cannabis.

My hope is that the doctor I will be working with will be able to see that my testosterone is decreasing now, year by year. As I have a track record now. However I do understand they can say no. I appreciate you helping me set expectations for this upcoming appointment. Thank you.

Good luck man!
Hope your doc is willing to work with you to sort this stuff out 👍

Yes - my test values are about the same - and I've similar symptoms I'm trying to figure out - mostly fatigue , ed, tiredness. I am making headway by changing my diet - my fatigue has lessened quite a bit happily, and my weight is dropping. How old are you if you don't mind me asking ?
