Fit for 40 - TRT Log

I figured I'd start a TRT log, now that I have a baseline and my first round of bloodwork. Currently prescribed 100mg of Test E(Hikma) and 60mg of desiccated thyroid.

For my baseline pre TRT bloodwork I had;

Testosterone - 15.59 nmol/L
Bioavailable Testosterone - 4.46 nmol/L
PSA - 0.282 ug/L
SHGB - 26.5 nmol/L
Hemoglobin - 142 g/L
Hematocrit - 0.413 L/L
Glucose - 5.5 mmol/L
Estradiol - 73.2 pmol/L
Prolactin - 13.3 ug/L
LDL - 4.28 mmol/L
HDL - 0.96 mmol/L
Cholesterol - 5.83 mmol/L
FREE T3 - 4.96 pmol/L

For my first round of bloodwork on TRT I had;

Testosterone - 29.79 nmol/L
Bioavailable Testosterone - 11.25 nmol/L
PSA - 0.364 ug/L
SHGB - 27.9 nmol/L
Hemoglobin - 153 g/L
Hematocrit - 0.454 L/L
Glucose - 5.7 mmol/L
Estradiol - 203 pmol/L
LDL - 3.24 mmol/L
HDL - 0.84 mmol/L
Cholesterol - 4.75 mmol/L
FREE T4 - 11.8 pmol/L
FREE T3 - 4.96 pmol/L

My diet has been much cleaner. I'm doing cardio 6 days a week, and weightlifting 3-4 days a week. I can see this reflected in my LDL levels. However I still struggle to raise my HDL, it actually went down. I am a heavy cannabis smoker, so I wonder is this the culprit?

My weight has been down and up, but overall the same. My focus has been on quality foods. I do see the difference in the mirror, I am definitely losing fat. What I notice most is my mood change and taste for life, again. I am motivated, more confident, and feel like a man. I am not tired and I don't have trouble getting out of bed. I am able to give my wife and children the best version of myself, daily.

I struggled for years with ED, severe depression, anxiety, and PTSD. Since starting TRT my ED is gone. I still experience depressive episodes, but nothing like before and not near as often. Notably, zero intrusive thoughts. My first response in a situation is no longer fight or flight, now its how can I navigate this situation with the coping tools I have.

As always, any input is appreciated. I'll post some pictures, workout's, food intake, and other updates as I go.
 
Hormones - all looks ok if you feel fine and clinic happy with numbers. Don’t worry about the higher e2 unless it causing high BP or gyno. Its very heart healthy…suppressing E2 is bad in the long term. That said its higher that expected at that test dosage. Probably need to focus on diet and reducing body fat to get the most out of your current dosage of TRT. Don’t let a dr talk you into an ai.

2 things to follow up with GP:
1. Your PSA is now over .350. Not a doctor, but my understanding that this the point/ number where doctors start to investigate prostate issues, including BPH. DHT compounds are likely not in your future.

2. It’s likely genetic so you probably cannot improve your HDL much (healthy oils walnut, olive, salmon, etc help) but your LDL is too high. You need to get that under 2.5 and preferably under 2.0 given your low HDL. You should ask your doctor, if you’re in a province that has private healthcare, for a calcium CT score to understand if you are collecting plaque in the office.

Take some fish oil, for your HDL/LDL levels both are not so great.
It’s been a work in progress, caused by years of diet neglect, unfortunately. Still a long ways to go. I carry excess fat around, that’s slowly going away. Reducing my body fat percentage is my number one priority right now.

I do take plenty of fish oil already tho. 10, 1000mg caps a day for blood pressure management.
 
I had a pretty solid chest and tricep day yesterday. It was:

Bench press - 5 sets of 10
Incline dumbbell press - 5 sets of 10
Incline dumbbell flies - 5 sets of 10
Push ups - 5 sets to failure

Tricep kickbacks - 5 sets of 10
Tricep extensions - 5 sets of 10
Skull crushers - 5 sets of 10
Diamond push ups - 5 sets to failure

Here’s a little shot of most of my home gym. Does anyone else only workout at home?

I train at home.
Free weights in the basement, big multi machine and cardio in the garage.

You’ll get there. It will take time.
 
Today was legs and shoulders;

Squats - 10 sets of 10
Lateral raises - 5 sets of 10
Rear delt raises - 5 sets of 10
Front delt raises - 5 sets of 10
Dumbbell shoulder press - 5 sets of 10

As my log continues, you’ll notice the leg exercises are short and sweet in my routine. My main form of cardio is on my assault bike, between 80 - 90 km’s a week. For this reason, I don’t fry my legs on “leg” days.

This was the best feeling.
Getting out of bed with a spring in your step and some drive to take on the day. Makes all the difference.
Glad to hear things are going well for you.
My own process has taken quite a while to get things dialed in ( still working on it ) but at least I am working with a thorough Dr.
 
I've literally gained everything in my basement over my short few years of efforts, and it's not particularly fancy. Much like you, I've got a rack for squats/benches, an incline bench and leg curl/extention attachment.

I did make a dip station which is invaluable for me, and rigged up some pulley systems for tricep pushdowns/one arm cable rows, things like that and pullup bars in three configurations (neutral grip, supinated etc) it's all cobbled together to make for a pretty great setup. If you're inventive and comfortable with construction, you can make some pretty cool systems with steel cable and pulleys etc that suit your own needs.

Dumbbells and rack completed. Very happy with this upgrade to the gym. Even some additional room for heavier dumbbells.

Had an appointment with the Dr yesterday.

We’re both very pleased with my recent round of bloodwork, and more so how I’m feeling. Based off of those two things, the Dr decided to leave the dose of testosterone at 100mg. I completely agree with him.

Thyroid performance stayed the same despite taking desiccated thyroid, based off the bloodwork. So we both agreed to increase the dose to 90mg a day.

No AI added. Dr agreed estrogen is healthy for the heart and many other parts of the body, including the brain.

Next appointment is in October.
